Barnabas Patient Voices is the patient group for Barnabas Medical Centre, Northolt. Membership is open to: all registered patients of the Practice; carers of registered patients; and members of Practice staff.

In order to register as a member of Barnabas Patient Voices we require you to complete a couple of simple questions and agree the group’s rules. This information will be reviewed and held on file; we do not require any other personal information (although you may optionally provide it).

Your registration will then be reviewed by the group admins for approval. When approved you will receive a confirmation email.

By submitting the registration form and joining Barnabas Patient Voices I understand that:
- I will receive an email from Barnabas Patient Voices to confirm receipt of this application and it’s approval.
- I will receive occasional mailings of interest and relevance to Barnabas Patient Voices in particular, and the NHS in general. This will include the minutes of Barnabas Patient Voices meetings and a monthly update.
- My details will only be used for the above purpose and will not be divulged to any third party except our parent body Barnabas Medical Centre admin team.
- I am entitled to attend any Barnabas Patient Voices meeting. Meeting times will be published on this site and in the minutes of meetings.
- As a member I am entitled to vote on any matter within Barnabas Patient Voices business including the Annual General Meeting.
- I may unjoin the group at any time.
