Membership Process

  1. Membership of Barnabas Patient Voices is open to all patients, carers of patients, and staff members of Barnabas Medical Centre (the Practice).
  2. Membership may be requested by completing the Membership Form on the Barnabas Patient Voices website at https://barnabasvoices.org.uk/membership-application/.
  3. Members of the Barnabas Patient Voices Facebook group at https://www.facebook.com/groups/barnabas.ppg who have answered the same questions as in the Membership Form are also members. (These questions are asked either when joining the Facebook group, or when first posting to it).
  4. The three key questions are:
    Q1. What is Your Relationship with Barnabas Medical Centre?
    [ ] Registered Patient
    [ ] Carer for Registered Patient
    [ ] Staff Member
    Q2. Which of our Doctors did you last speak with?
    Q3. The postcode at which you are registered with the Practice
    These questions are designed to determine, to a first level of accuracy, that the applicant is registered with Barnabas Medical Centre.
  5. For patients with email, the responses to Q1 and Q3 above, along with name and email address is all that will be retained in the membership register.
  6. For those without email seeking membership, a note answering the above questions and including the applicant’s mailing address should be sent to the Barnabas Patient Voices Chairman, c/o Reception at the Practice. (In this instance mailing address is required in order to be able to send Monthly Members Updates etc.). This information will be held in the Membership Register.
  7. All Practice staff are considered to be members of Barnabas Patient Voices. However their NHS email addresses will be recorded in the Membership Register (and thus they will receive group communications) only where permission has been granted.
  8. [Note some early members of Barnabas Patient Voices, who may not have answered the three questions, will have been included in the Membership Register via a “grandfathering process”.]
  9. From time to time (and at least annually) the full list of patient and carer members will be checked with the Practice to ensure they are/remain registered with the Practice. Those who are not registered will be removed from the membership list.
  10. The full list of members will be held by the Chairman, with copies held by the Vice-Chairman and the Practice Manager.
  11. Members’ details may be shared with Barnabas Patient Voices elected officers and the Practice’s admin team. Member’s details will not otherwise be shared or disclosed unless legally required or the member has given explicit permission.
